The measurement of serum levels of etoposide using high performance liquid chromatography was investigated in patients with small cell carcinoma of the lung. 19 patients were treated with two cycles of etoposide (600 mg/m2 daily) on three days as a six hour infusion, 2 patients were received modified schedule with the same dose given as a continuous 72 hour infusion during the second cycle. Peak levels of etoposide ranged between 17.5 ug/ml and 66.0 pg/ml (mean 43.0 ug/ml) during cycle 1 and between 22.5/ml and 66.0 ug/ml (mean 45ug/ml) for cycle 2. Variation was seen in the area under the concentration curve of the drug (AUC) with the range for the course 1 being 698 to 1799 g/ml/h and for cycle 2 1054 to 1803 pg/ml/h. The AUC for the second cycle of treatment in the 2 patients who received the drug as a continuous infusion were 44% and 66% that obtained during the first cycle of treatment. The median survival of the patients in the trial was 262 days with a single patient surviving over 1500 days. No correlation was found between dose of drug, plasma levels of etoposide or AUC values with survival or response to treatment. Severe dose-limiting toxicity (neutropenic septicaemia) was seen in all patients and was most severe following the first treatment. No correlation was seen with toxicity and plasma levels or dose of etoposide. The measurment of drugs used in the treatment of cancer is being increasingly advocated, however, measurement in plasma may prove unsatisfactory since it may not reflect drug levels or activity at the target site.
